Preserving tissue instead of cutting through it
The name says it all: preservé means “preserved”. And that is exactly the guiding principle. In a conventional breast augmentation, the implant pocket is created by cutting – through skin, connective tissue and often the chest muscle as well. Preservé™ reverses this approach.
Through an access of only about 2.5 cm in the fold beneath the breast, the tissue is stretched gently and in a controlled way with a special balloon until the pocket for the implant has formed. Muscles, nerves, blood vessels and the supporting ligament structures of the breast are preserved. Which means: less trauma, less pain, significantly shorter downtime.

The procedure
A day of surgery, planned like a good appointment
At W Clinics, the day of your procedure is not a hospital marathon but a structured, calm process. You arrive in the morning, go over all the details once more with your surgeon and are then accompanied to the operating theatre.
The treatment itself usually takes 30 to 45 minutes. You decide on the form of anaesthesia – local anaesthesia with twilight sedation or general anaesthesia – together with the anaesthesia team in advance. After a short recovery phase, you can usually leave the clinic on the same day.

At the heart of the Preservé™ method is a system of three single-use Motiva® instruments developed specifically for this procedure. Each has a clearly defined task and works with the tissue, not against it.

01 · Channel Separator
Creating the access
With a 2 cm wide, blunt tip, the Motiva® Channel Separator clears a path through a minimal incision to the future implant pocket. Using a circular rotating movement, it loosens the inner tissue without injuring it – the supporting structures of the breast remain intact.

02 · Inflatable Balloon
Stretching the tissue, not cutting it
The inflatable Motiva® balloon shapes the implant pocket through controlled, gradual expansion. Tissue and ligament structures are pushed radially aside – without cutting, without trauma. The result is a precise 3D space that fits the future implant shape perfectly.

03 · Insertion Sleeve
Safe, sterile, touch-free
The implant is inserted via the sterile Motiva® Insertion Sleeve without coming into contact with skin or gloves. This no-touch technique shortens the operating time, allows smaller incisions and minimises the risk of infection.

What to expect after the procedure
Because no tissue is cut, recovery is usually straightforward. In the first few days, slight swelling and a feeling of tightness are normal, but mostly well tolerated. A support bra helps maintain the shape during the first weeks. The final breast shape develops over several months – after about six months, the result is usually stable.

Who is Preservé™ suitable for?
Who benefits most from the method
Preservé™ is aimed at women who want a breast augmentation but cannot or do not want to accept much downtime – active, working women as well as mothers with young children. The procedure is particularly well suited to primary augmentations, i.e. first-time procedures. Women who plan to have children also benefit from the gentle tissue handling, because glandular and nerve structures are largely preserved and the ability to breastfeed is usually not impaired.
Preservé™ is less suitable in cases of very pronounced skin laxity or if a breast lift is planned anyway – other procedures are more appropriate here. FAQ: Preservé™ breast augmentation at W Clinics
How exactly does Preservé™ differ from a conventional breast augmentation?
The key difference lies in how the implant pocket is created. In a conventional breast augmentation, tissue is cut with a scalpel – muscle, connective tissue and often nerve pathways as well. Preservé™ replaces this step with controlled tissue stretching using an inflatable balloon. The result is significantly less trauma, a shorter recovery and the preservation of sensation and support structures.
Is the procedure painful?
You feel nothing during the operation – thanks either to local anaesthesia with twilight sedation or to general anaesthesia. In the days afterwards, most patients report a feeling of tightness and mild pain that can be well controlled with standard painkillers. The difference from the conventional method is often clearly noticeable here, because no muscle or nerve structures have been injured.
Do I need general anaesthesia for Preservé™?
No – one of the advantages of the procedure is that it can also be performed without general anaesthesia. Many of our patients choose local anaesthesia with twilight sedation, a relaxed, sleep-like state under the supervision of the anaesthesia team. General anaesthesia is of course also possible if you feel more comfortable with it.
When can I return to work?
That depends on your job. With an office job, many patients are back at work after 3–5 days. For physically demanding work, we recommend a break of at least two to three weeks. Light exercise is often possible after just one week, intensive training after three to four weeks.
Which implants are used?
Exclusively the latest SmoothSilk® Ergonomix2® implants from Motiva. They are characterised by a particularly soft feel, movement that adapts to the body and integrated Zen® RFID technology, which allows the implant to be identified at any time – including in an MRI scan, without an additional procedure.
Is sensation in the breast retained?
Because no nerves are cut with Preservé™, sensitivity is, as a rule, fully retained. This applies both to sensation in the breast tissue and to the nipple.
Can I still breastfeed after a Preservé™ breast augmentation?
Yes, as a rule the ability to breastfeed is not impaired. The glandular tissue and the nerve structures that supply it are not affected by the tissue-sparing technique. This is why the procedure is also well suited to women who plan to have children.
How visible will the scar be?
The access lies in the natural fold beneath the breast and, at approx. 2.5 cm, is very small. Once fully healed, the scar is barely visible in most patients. Like any scar, it matures over several months and fades over time.
Who is Preservé™ less suitable for?
Preservé™ was developed primarily for first-time procedures. In cases of very pronounced skin laxity, or for patients who also need a breast lift, other procedures may be more suitable. Whether Preservé™ suits you is something we clarify in the consultation, based on your individual anatomy.
